Summary:
Provides comprehensive health care services to a caseload of home health patients and their families through direct care and supervision of home health aides and collaboration with other disciplines involved in care. Assesses patient's physical and mental status and clinical care needs including the impact of their environment, and social support, Develops and implements the initial treatment plan and modifies according to changes in patient condition and or in collaborative with the physician.

Minimum Education:
- Graduate of an accredited school of professional nursing, or a school accepted for candidacy status from an accrediting body.

Minimum Work Experience:
- One (1) year of clinical experience in the area of specialty providing professional nursing care preferred

Required Skills/Knowledge:
- Current license to practice nursing in the State of Ohio.
- Valid Ohio driver's license with safe driving record and proof of insurance.
- Maintains confidentiality of all patient information.
- Effective communication skills.
- Assures that patient rights are maintained at all times. Reports any violations or suspected deviations according to policy.
- Ability to follow safety, security, infection control (Body Substance Isolation) and hazardous materials policies and procedures. Assures patient, personal, and coworker safety.
- Shows flexibility/adaptability in meeting environmental constraints and demands placed on services as is common with changing reimbursement and managed care targets.
- Demonstrates effective time management with daily schedule, individual treatment sessions, and non-patient care.
- Population Specific Competency: Ability to effectively interact with populations of patients/customers with an understanding of their needs for self-respect and dignity.
