Telephone Triage Nurse

Summary of Responsibilities

Performs telephone triage of patients, and implements appropriate course of action via established triage protocols or in consultation with physician.

Essential Functions

1. Conducts outbound calls to patients, and uses nursing process to assess and document the patient's signs and symptoms, taking into account physiological, cultural, psychosocial, and environmental factors and patent/family educational needs.
2. Chooses correct triage protocol and disposition based on patient information gathered, and gives care advice in accordance with established protocols.
3. Documents phone call information designated computer software application, in timely and accurate manner.
4. Participates in quality assessment and improvement projects to evaluate and improve performance. 5. Establishes and maintains positive working relations with patients, physicians, and customers.
6. Meets all established competency performance standards.
7. Performs all job functions with integrity. Provides timely internal and external customer service in cooperative, professional and respectful manner.

Educational Requirements

Associates Degree or Diploma in Nursing from an accredited nursing program, or may substitute with 2 years experience working in same or similar job title.

Required Experience

3+ years performing same or similar responsibilities. Relevant experience includes: direct patient triage, clinical phone triage, demonstrated excellence in clinical assessment using the nursing process, or adult/pediatric nursing care in a physician or hospital setting. Excellent communication and interpersonal skills also required.

Registered Nurse License (RN) in State of Ohio
