Job Summary:
Assists individuals with serious mental illness and those who are dually diagnosed with the development of an individualized comprehensive personal plan and designs a path to achieve these goals. Institutes an illness management and recovery process that encourages independence and reduces crises. Provides practical information, strategies and skills that will achieve the recovery goals. Works with the Community Support Program Services Team in implementing the treatment/service plan. Monitors services provided by other staff and/or agencies in assisting clients towards their goals of independence.

Minimum Qualifications
Educational/Training Requirements:
Bachelor's degree in the Social Sciences. Graduate degree preferred.

Experience:
2-5 years relevant community experience.

Licenses/Certifications:
Maintain both a valid Ohio Drivers License, and a driving record that meets JFSA's insurance company standards (eg less than 2 incidents over a 3 year period)
Maintain personal auto liability insurance at the level that the Agency and the insurance carrier require.

Knowledge, Skills and Abilities:
Interest and desire to work with SMI/DD populations.
Strong written and verbal communication skills.
Knowledge of behavioral health psychopathology & disabilities.
Good problem solving skills.
Good interpersonal skill.
Ability to multitask effectively, manage stressful situations, work within a team concept.
Good time management skills.
Good organizational skills.

Physical Demands are minimal and typical of similar jobs in comparable organizations.
Work Environment is representative and typical of similar jobs in comparable organizations.
Must have personal vehicle available on a daily basis for client transport.
