Support Blood Pressure, Cholesterol Improvement, and Diabetes Control work with healthcare providers in the clinic setting to ensure standard methodologies for the treatment of patients through initiatives and programs, for instance Target BP, Check. Change. Control. CHOLESTEROL and Know Diabetes By Heart. Collaborate with clinical staff for data entry, data reporting, compliance issues, and strategies for improved patient outcomes.
Drive Collaboration Drive collaboration with development staff in the market around mission-related goals and work with internal staff partners to ensure population health strategies are coordinated into development, communications and marketing, advocacy, and healthcare quality improvement activities. Secure program funding, including sponsorship, for relevant campaigns. Support sustainable change within an organization by taking a comprehensive approach, including a focus on policies, systems and environments, to ensure equitable health for all.
Engage & Empower Volunteers Be accountable for the recruitment and management of volunteer networks. Build a network of relevant volunteer partnerships to advance the mission. Provide timely direction, meaningful opportunities, resources, and recognition to volunteers while at the same time relying on their expertise, abilities, and willingness to use their networks to drive the goals of the organization. Hold each other accountable, both volunteers and staff while being accessible and build collaborative staff-volunteer partnerships that are based on a foundation of mutual trust.


In this role, you will report to the Vice President of Health Strategies. Daily travel within the market is required (when safe to do so). Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

Here are some of the requirements
Bachelors degree and 5+ years of demonstrated ability working in community/public health.
Demonstrated critical thinking skills in assessing the local market environment and available resources to successfully implement new and existing programs.
Proven ability to recruit, mobilize, and manage volunteers, including C-suite level executives.
Demonstrated ability to manage large projects and events ensuring target compliance.
Demonstrated ability to efficiently participate in a multi-disciplinary team environment working towards common goals with internal and external clients. Ability to influence team members without supervisory authority.
Excellent listening skills, including verbal and non-verbal communication, active listening, negotiating, problem-solving and decision-making
Highly effective organizational skills, time management, responsibility, leadership, and motivation
Understand the benefits associated with the use of treatment algorithms and their impact on clinical inertia.
Experience working with multicultural communities preferred.
Ability and willingness to occasionally travel outside the market and to work evenings and weekends as needed.
