Welcome Health delivers home-based primary care directly to older adults in the greater Los Angeles area. Founded in the science and art of geriatric medicine, the team at Welcome Health believes that the best primary care is anchored on what matters most to each person. Our care model recognizes the unique needs of older adults. In addition to preventative care and chronic disease management, we focus on aging-related issues that significantly impact the quality of life of our seniors, such as mobility limitations, cognitive impairment, incontinence issues, and polypharmacy.

In our physician-led care model, physician assistants partner closely with physicians to implement the care plan for each patient within a shared patient panel. Because we care for home-bound patients who otherwise have difficulty getting to the doctor, the majority of the work associated with this position will occur in the field, caring for patients in their homes.

How You Will Help Our Patients

Welcome Health PAs are an integral part of our physician-led, multi-disciplinary teams. To thrive in this environment, you will take pride in:

  • Treating our teammates with the same care you treat our patients, using your best communication, collaboration, and problem-solving skills to help make us the planets best medical team - including medical assistants, physicians, community health workers and others.
  • Taking the time to understand the full context of our patients lives, with an eye toward their mental, physical, and social health, as well as the role family and loved ones play in their well-being.
  • Taking initiative as a physician assistant while working closely with our physician leaders to coordinate the best care possible. Proactively engaging patients to monitor their care plan progress.
  • Carefully guiding patients on their prescriptions and treatment plans (including family members when appropriate), so they feel invested and supported in their progress.
  • Pursuing mastery - with the active support of our physician leaders - in caring for common chronic diseases in older adults, including (but not limited to) diabetes, hypertension, heart failure, chronic kidney disease, wound care, and behavioral health disorders.
  • Identifying and addressing gaps in preventive care, including annual and age-related screening tests, vaccine requirements, fall risk screening, depression screening, and cognitive-function screening.
  • Calling patients to discuss - with utmost empathy - test results and prognoses.
  • Interpreting medical records and reports and, if necessary, referring patients to specialists.
  • Completing follow-up and post-hospital assessments according to documented standard operating procedure.
  • Managing or triaging urgent medical issues.
  • Pursuing and demonstrating best practices in core nursing procedures, including IV placement, blood draws, injections, foley catheters, nasal packing, wound care, etc.
  • Utilizing the Electronic Health Record (EHR) and updating patient charts in timely completeness and team-friendly language.
  • Bringing exceptional listening and interviewing skills to patient interactions, including physical exams, medical history and symptoms.
  • Confidently working with our software systems, including telemedicine, scheduling, database, research, diagnosis, billing, and e-prescribing.
  • Bringing your insights and listening skills to our meetings, including (but not be limited to):
    • Daily huddles
    • Weekly case conference reviews
    • Monthly staff meetings
    • Joint Operating Committee meetings
    • Quarterly Quality Improvement Committee meetings
    • Continuing education/journal club programs
  • Performing other duties as necessary to achieve our patient care mission

What Youll Need to Succeed as a Welcome Health PA

  • Graduate of an accredited Physician Assistant program
  • California PA License Required
  • Current DEA
  • The ability to travel within your assigned area and physical ability to provide home-based primary care in a variety of residential settings
  • Experience and comfort (+) in visiting patients in their homes
  • A reliable vehicle for patient visits
  • Experience and comfort in working with electronic health records and mobile technology
  • Comfort working with Microsoft Teams and Microsoft Outlook
  • At least two years of experience evaluating, assessing, and providing patient care are preferred
  • Fluency in a second language such as Spanish, Korean, Armenian, or Mandarin Chinese is preferred
