Reporting to the Executive Director and is responsible for daily monitoring and appropriate intervention for patients who are receiving remote in-home monitoring of vital signs and question set trigger triage. Reviews data, triages phone calls, informs physicians and Primary Care Nurse of vital sign trend, clinical concerns per established protocols to insure safe and appropriate patient care.

Minimum Qualifications:

• Bachelor's degree in nursing
• RN Licensure in the State of Connecticut.
• Must be proficient computers (ie Google Docs and Worksheets, Microsoft Word and Excel).
• At least one year of clinical nursing in telemetry or adult critical care.
• Must have the ability to interpret ECG rhythm strips.
• Previous experience with clinical information systems.
• Excellent telephone communication skills.
